Copyright Permissions Form Letter Instructions
· Mouse over the form letter for fields that need to be edited
· Double-click on the blue highlighted field
· Edit information as it pertains to your program and copyright request
· To save a copy of your completed copyright permissions letter, click on “save as” and rename your document.
